چکیده

Precordial ST-segment mapping was applied serially in the coronary care unit for the study of 46 patients with myocardial infarction (MI), using a 49-lead system. Data from the maps were compared with clinical status of patients, conventional ECGs obtained simultaneously, and serum enzyme levels. Stability of the maps over a one hour period was noted in the early phase of admission. However, a drop of 32% of the sum of ST-segment elevations (+ 2 ST) was detected in eight patients with uncomplicated anterior MI over the first 24 hours after admission. Extension of infarction was associated with abrupt rise of + 1 ST, and was diagnosed in two cases from maps in the presence of unchanged standard ECGs. The course of ST elevations was followed more accurately by the map than the standard ECG in eight patients. Pericarditis invalidated the technique completely, due to persistent + 1 ST. The standard ECG was superior to the map in following patients with inferior MI. A case of true posterior MI was more accurately delineated by maps of the posterior thorax than by the standard ECG. Intraventricular conduction defects and pacemaking interfered with the maps. Early repolarization produced stable maps; however, mapping showed no advantages over the standard ECG. Preinfarction angina can probably be followed by serial mapping of ST-segment depres-

Body surface potential mapping in patients with Brugada syndrome: right precordial ST segment variations and reverse changes in left precordial leads.

OBJECTIVE The aim of this study was to perform quantitative signal analysis of high-resolution body surface potential mapping (BSPM) recordings to assess its usefulness for the electrocardiographic characterization of patients with Brugada syndrome. The diagnostic value of the QRS integral and of the gradient of the ST segment have not been elucidated in Brugada syndrome. METHODS In 27 subjec...

Electrocardiographic Precordial Mapping in Anterior Myocardial Infarction

Serial 72-point precordial mapping of the ECG has been recorded to describe the natural history of changes in the precordial areas of ST segment elevation and the development of Q waves in 51 patients with acute uncomplicated anterior myocardial infarction. Eight patients have been studied in the same way but received 25 mg/kg of methylprednisolone sodium succinate as a single intravenous injec...

Evaluation of precordial orthogonal vectorcardiographic lead ST-segment magnitude in the assessment of myocardial ischemic injury.

Relationship has been established between epicardial ST-segment elevation, considered a reliable estimate of ischemic injury in experimental myocardial damage, and ST changes by multiple-lead precordial electrocardiography. However, 35-lead precordial mapping is time-consuming and suitable only for anterior infarctions. An alternate, more rapid method for recording ST segments is an external 3-...

Prognostic significance of precordial ST segment depression during inferior myocardial infarction in the thrombolytic era: results in 16,521 patients.

OBJECTIVES We examined the prognostic significance of precordial ST segment depression among patients with an acute inferior myocardial infarction. BACKGROUND Although precordial ST segment depression has been associated with a poor prognosis, this correlation has not been adequately quantified, partly because of small sample sizes and methodologic limitations in previous studies. METHODS W...
